Clinical effectiveness of diode laser therapy as adjunct to non-surgical periodontal treatment: a randomized clinical study

The aim of this randomized clinical study was to evaluate the effect of a 980 nm diode laser as an adjunct to scaling and root planing (SRP) treatment. Thirty-five patients with chronic periodontitis were selected for the split mouth clinical study. SRP was performed using the sonic device and hand instruments. Quadrants were equally divided between the right and left sides. In two control quadrants (CG) teeth were treated with SRP, and in contralateral quadrants, the diode laser was used adjunctively to SRP (LG). Diode laser therapy was applied to periodontal pockets on the 1st day following the SRP, and then on the 3rd and 7th day. Baseline data, including approximal plaque index (API), bleeding on probing (BOP), probing depth (PD), and clinical attachment level (CAL) were recorded before the treatment and at 6 and 18 weeks post-treatment. Changes in PD and CAL were analyzed separately for initially moderate (4-6 mm) and deep pockets (7-10 mm). The results were similar for both groups regarding the API, BOP, PD high and CAL. Laser group showed only significant PD gain in moderate pockets during the period baseline-18 weeks (p<0.05) and 6-18 weeks (p<0.05), whereas in remaining clinical parameters, no difference between laser and control groups was found (p>0.05). The present study indicates that multiple adjunctive applications of a 980 nm diode laser to SRP vs SRP alone showed improvements only in moderate periodontal pockets.
