THE ROLE OF EXTERNAL QUALITY ASSESSMENT IN ACCREDITATION OF MEDICAL LABORATORIES IN CROATIA
The main goal of medical laboratories is to be able to support the clinicians with the best achievable quality in all laboratory results and reports. Ongoing evaluation and improvement processes are essential to prove laboratory competence to provide performance incompliance with the highest professional standards having optimal health benefit for the patient. Two main external quality assessments are used for this purpose: accreditation according to the international and European standard ISO 15189, Medical laboratories– Particular requirements for quality and competence’ and proficiency testing schemes – PT, also called external quality assessments – EQA. According to requests of ISO 15189 laboratory must have evidence of an effective quality system what can be accomplished by participating in quality examination schemes (PT/EQA). Laboratory is responsible for the handling with PT/EQA samples in the same way as with patient’s samples and analysis of unacceptable results. Croatian Accreditation Agency (HAA) as a national accreditation body requires laboratories to inform the HAA in written on participation and achieved results in PT/EQA schemes in order to demonstrate technical competence in the areas which are included in their scope of accreditation. Proficiency testing programs in Croatia have been continuously performed since 1973, by the Committee for External Quality Assessment, today CROQALM – Croatian centre for quality assessment in laboratory medicine as a part of the Croatian Society for Medical Biochemistry and Laboratory Medicine. The role of the national EQA scheme is to establish objectively the degree of interlaboratory comparability and contribute through harmonization and standardization of laboratory methods as well as continuous education to improvement of laboratory medicine in Croatia. In order to improve the organization and management of the national EQA schemes CROQALM introduced new software called INlab2* QALM – application for quality assessment in laboratory medicine, capable to receive and handle our participant’s orders, results, method changes, invoices, reports and notifications to participants, with all CROQALM schemes in electronic form. The schemes include general and special medical biochemistry, laboratory hematology and coagulation and are conducted three times a year for about 200 medical biochemistry laboratories in Croatia. The obtained results which are evaluated by using participants consensus values showed that most of the participants fulfill the desirable analytical quality specifications base on biological variation as a pre-requisite for harmonization of laboratory results on national level. According to the norm ISO 17043 for PT/EQA providers one of the most important feature is to focus on the highest quality of control material which has to be commutable, stable and homogenous. According to this requirement in the Croatian EQA scheme for laboratory haematology, home-made samples from a single donor prepared by the Croatian Institute for Transfusion Medicine are used for evaluation of the complete blood count. The obtained results show applicability to all haematological analyzers and provide a long term, retrospective assessment of laboratory performance. The obtained results show that proficiency testing (PT/EQA) programs in Croatia, conducted through CROQALM, have an important role in improving the quality of laboratory results enhancing their clinical efficiency and raising the quality of health care.
