Hybrid abutment-crown implanto-prosthetic solution in the aesthetic zone

Background: Retention of prosthetic part to the implant is divided on the screw and cement retained one. Both possess certain advantages and disadvantages. In order to eliminate the disadvantages of the two concepts and bring out their benefits, a hybrid abutment-crown concept was introduced. Aim/Hypothesis: The aim of this presentation was to describe the advantages of the hybrid abutment-crown implanto-prosthetic concept on two cases in the aesthetic zone. Material and Methods: First patient, a 27-year old female, came to our Department with an acute periapical process of the upper right second incisor. Tooth had already been apicoectomied twice and the patient insisted on having it extracted. After extraction, socket preservation was performed and a temporary Maryland bridge provided. An implant was placed and 4 months later an individual temporary crown was designed which also served as a sulcus former. Individualizing the transfer enabled maximal reproduction of the obtained gingival emergence profile. Implant was loaded with a hybrid abutment-crown restoration 5 months after implant placement. Second patient, a 37 year-old female, had her both upper canines extracted in her adolescence. She underwent an orthodontic treatment and space for implants in the upper canine regions was provided. We present a implanto-prosthetic treatment using a hybrid abutment-crown concept. Surgical and prosthodontic protocols for the hybrid abutment-crown concept are depicted and elucidated. Results: Both cases showed satisfactory aesthetics and functionality after a 2 year follow up. Conclusions and Clinical Implications: Hybrid abutment-crown concept enables high prosthetic individualization of the developed emergence profile. Furthermore, it possesses the advantages and diminishes the disadvantages of a cement and screw retained dental abutments.
